We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

HOW TO ENROLL

Action Required: All elections must be submitted by 03/29/2023


ENROLLMENT INSTRUCTIONS:

  1. Log in to askar.ease.com per the instructions you have received from HR
  2. Log in to Ease per the instructions you have received from your HR administrator
  3. Follow instructions and enroll in your benefits
  4. Make sure to save your elections



Remember, this is the only time during the year that you can enroll in coverage or change your benefit elections, unless you have a qualifying event during the year. You have 30 days from the qualifying event date to notify your Benefits Administrator to make the changes. Common life events are marriage, divorce, birth, adoption, and loss in coverage.

You have three options to choose from:


  • BCBSM PPO 1500
  • In and Out of Network Coverage available.
  • Members are not required to have a designated PCP to receive in-network benefits.
  • No referral is required to see a specialist.
  • BCN POS 1000
  • In and Out of Network Coverage available.
  • Michigan residents are required to have a designated PCP to receive in-network benefits. In network providers include BCN HMO providers.
  • Non-Michigan residents do not need to select a PCP. In network providers include nationwide Blue Card Traditional providers.
  • No referral is required to see a specialist.
  • BCN POS 3000
  • In and Out of Network Coverage available

Visit BCBSM.com under “Find a Doctor”. Set your location and select “PPO Plans” under plans.


POS - Provider Search

Michigan Member: Visit BCBSM.com under “Find a Doctor”. Set your location and select “Blue Care Network (HMO and POS)” under plans.

Member Outside of Michigan: For BlueCard providers, go to provider.bcbs.com to set your location and select “BlueCard Traditional”

Askar Brands provides those employees who are covered by one of the medical plans access to virtual visits with BCBSM Online Visits. BCBSM Online Visits provides members the opportunity to receive medical and mental health care from the comfort of your own home. With BCBSM Online Visits, eligible employees and dependents have access to board certified doctors, psychiatrists and licensed therapists 24 hours a day, 7 days a week, 365 days per year.


Common Medical Services Include:

  • Bladder Infections
  • Cold/Flu Symptoms
  • Sore Throat
  • Pink Eye Rashes
  • Sinus Infections
  • Ear Infections
  • Stomach Issues


How Much Does it Cost?

  • PPO 1500: Covered 100%
  • POS 1000: Covered 100%
  • POS 3000: Covered 100%
  • POS HSA 6900: Coverd 100% After Deductible (Prior to deductible, member will be responsible for a $59 charge that will apply towards deductible) 
  • Prescriptions: If you are prescribed a medication during a virtual visit, applicable Rx cost sharing will apply based on which medical plan you are enrolled in and whether your deductible has been met.

Askar Brands offers Vision coverage through Unum to help pay for eye exams, prescription glasses and contact lenses. You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations.
