We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.
Eligibility
Regular employees working at least 30 hours per week are eligible to participate in benefits offered by Friendly Chevrolet. When you enroll in any of the plans you may also cover your eligible dependents, including your legal spouse, child(ren) up to age 26 (regardless of student status, marital status, residence, or financial dependence on you), and an unmarried child incapable of self-support.
New Hires
The benefits you elect during your new hire eligibility window will be effective the first of the month after 60 days of employment and remain in effect until our annual enrollment in November, for a December 1st effective date.
Annual Enrollment
Friendly Chevrolet's annual enrollment is held in November for a December 1st effective date. All benefit-eligible employees can make changes to their elections at this time.
You can, however, modify your elections under certain circumstances, called "Qualifying Events." These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child. All employees will be electing their benefits through our new online enrollment platform called Ease. Click on the Ease button to be directed to the Ease Login page.
Friendly Chevrolet offers two plans with HealthPartners; an H.S.A. qualified medical plan that features a $4,000 single / $8,000 family deductible and a $1,000 deductible $40 copay plan, both with the Open Access Network or the Select Network. You have the freedom to use in-network and out-of-network providers; however, your out-of-pocket expenses are much lower when you use in-network providers.
If you choose to enroll in the HSA qualified plan, you may be eligible to open and contribute to a Health Savings Account (HSA). See the HSA section for more information.
Friendly Chevrolet offers a dental plan through HealthPartners that allows you to seek treatment from either in-network or out-of-network dentists. If you choose to seek care outside of the HealthPartners Dental Network, you may end up paying more out of your pocket. Seeing a dentist that is part of the Open Access network allows you to take advantage of the lower fees that HealthPartners has negotiated.
Preventive care is covered at no cost to you when you see a network dentist. Services such as getting a cavity filled might be subject to the deductible first, which is the amount you have to pay before your plan helps with the cost. There's also coinsurance, which is a percent of the bill.
The plan's annual maximum is a bit different than the out-of-pocket on your medical plan. It's the most your plan will pay for dental care each year. You’re in charge of the other costs you may incur.
The Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible expenses. By contributing to a HSA, you reduce your taxable income and pay less in taxes which saves you money.
To be eligible for an HSA you must be enrolled in a High Deductible Health Plan (HDHP), such as the HealthPartners $4,000-100% plan, and you cannot have any other coverage other than that qualified HDHP. The money you contribute rolls over year after year and remains with you even if you change jobs. There is no "use it or lose it" clause with an HSA. Friendly Chevrolet works with Optum Bank, who is the holder of your HSA funds.
The maximum contributions for calendar year 2023 are:
$3,850 for a single contract
$7,750 for a two person or family
The maximum contributions for calendar year 2024 are:
$4,150 for a single contract
$8,300 for a two person or family

Friendly Chevrolet will pay 100% of the employee premium. Employees can choose to cover dependents and will pay that premium through payroll deduction.

Friendly Chevrolet provides you with Basic & Voluntary Life/AD&D coverage as well as Voluntary Short- and Long-Term Disability through Mutual of Omaha.
Basic Life/AD&D
All benefit eligible employees receive $50,000 of life insurance coverage payable to your designated beneficiary in the event of your death. An additional accidental death & dismemberment benefit (AD&D) is payable to you in the event of a covered dismemberment or to your beneficiary if your death is the result of an accident. This benefit is paid by Friendly Chevrolet on behalf of all eligible employees.
Voluntary Life/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, you may also elect insurance for your spouse and/or dependent children. As a new hire, you can elect up to a certain amount ($100,000 for employees, $30,000 for spouses) without having to provide evidence of insurability.
Changes in your amount of insurance during the year, or during the annual open enrollment, may require that you satisfy health underwriting:
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Voluntary Short-Term Disability
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills. Your coverage through Mutual of Omaha pays 60% of your income to a maximum of $1,000 per week after a 7-day elimination period if you are deemed disabled.
You are able to enroll in this plan during this enrollment period even if you declined coverage in the past.
