Helping you and your family reach and maintain good health is very important to Preh, Inc. Preh, Inc. is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.


Preh, Inc. remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees. Our plans will remain the same for the 2022-23 benefit year. Please note, there has been an adjustment to the PPO medical plan employee contributions and you may be subject to changes in cost of your voluntary life/ad&d insurance if you have moved into another age bracket.

Your 2022-2023 Benefit Guide

ENROLLMENT

All regular employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Preh, Inc. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children. Children eligibility is as follows -


  • Medical, Dental, & Vision: End of the year the child turns 26
  • Voluntary Life and Critical Illness: Age 19 or 25 if full time student


Action Required: All employees must submit one election form to Jennifer by 07/28/2022.


ENROLLMENT INSTRUCTIONS:

  1. Review all of your benefit information so that you can make informed decisions for your 2022-23 elections.
  2. Complete your 2022-23 Enrollment Form.
  3. Complete the 2022-23 Full Enrollment Form if you need to make changes to your current elections - change plans, enroll in coverage that you previously waived, and/or make changed to your covered dependents.
  4. Complete the 2022-23 No Changes/FSA Form if you wish to maintain current elections. If you wish to participate in the FSA for 2022-23 plan year, be sure to complete the FSA section as well.
  5. Return your completed form to Jennifer by 7/28/2022.


The Benefit choices you make during your open enrollment remain in effect for the entire year.


You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

Preh provides those employees who are covered by one of the HAP medical plans access to virtual visits with American Well (Amwell). Amwell provides members the opportunity to receive medical care rom the comfort of your own home. With Amwell, eligible employees and dependents have access to board certified doctors 24 hours a day, 7 days a week, 365 days per year.


If you are enrolled in the HAP PPO, there will be a $30 copay for the virtual visit. If you are enrolled in the HAP HMO, there will be a $20 copay for the virtual visit.

You receive a higher level of benefits when you see a provider in network, however, out-of-network coverage is provided but may only be handled as reimbursements in some situations. Preh, Inc. provides you with financial protection options for life, accidental death & dismemberment, short term disability and long term disability with OneAmerica.


Life/AD&D: A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.


  • Basic Life/AD&D Employer Provided Benefit - No Cost to You
  • $50,000 Life/AD&D Benefit | Reduces 35% at age 65 and 50% at age 70
  • Voluntary Benefit - Costs are based on Age and Amount of Coverage
  • Employee: $10,000 increments up to the lesser of 5 x income or $250,000
  • Spouse: $5,000 increments not to exceed the lesser of your amount or $125,000
  • Child: $10,000


Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.


Short Term Disability: Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.


  • STD Employer Provided Benefit - No Cost to You
  • Benefits will begin on the 1st day of an accident and on the 8th day of an illness (subject to an approved claim)
  • 60% of your weekly earnings up to $1,000 weekly benefit
  • Benefit is payable for up to 13 weeks


Long Term Disability: Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household. 


  • LTD Employer Provided Benefit - No Cost to You
  • Benefits will begin on the 91st day of a qualifying accident or illness
  • 60% of your monthly earnings up to $10,000 monthly benefit
  • Benefit is payable to Social Security Full Retirement Age, a reduced benefit duration will apply if you are still actively working upon your retirement age
  • Pre-Existing Condition Limitation 6/12: Benefits will not be paid if the disability begins in the first 12 months of coverage and the disability is caused by, contributed to by, or the result of a condition for which medical treatment was received in the 6 month period just period to the effective date of coverage.

The Health Care and Dependent Care Flexible Spending Accounts (FSA) allow you to set aside pre-tax dollars to pay for eligible expenses. By contributing to one or both of the Flexible Spending Accounts you reduce your taxable income, so you pay less in taxes — which saves you money.


Contributions

The election you make during enrollment is your election for the entire plan year. You may change it only if you have a qualifying life event and the change request must be consistent with the event.


You may contribute as follows:

Health Care FSA

  • Up to $2,850
  • All benefit eligible can participate
  • Reimbursements allowed for unreimbursed medical, prescription, dental, and vision expenses


Dependent Care FSA

  • Up to $5,000 annually
  • Limited to $2,500 if you are married and file separate tax returns


Applicable to Health Care FSA & Dependent Care FSA

  • Grace Period: At the end of each plan year, you may use these funds left over from the current years FSA balance to pay for eligible expenses incurred in the first 2 ½ months of the following plan year (until 10/15/2022). Any Unused funds after the 2 ½ month grace period has been exhausted will be forfeited. *During the Plan’s Grace Period, do not use your FSA debit card for these expenses.
  • Runout Period: You have 60 days after the Grace Period ends to submit claims for expenses incurred during the previous plan year.

Critical Illness coverage can help ease the financial burden of surviving a critical illness by providing financial resources to help with medical costs or ongoing living expenses. This plan can help you focus on recovery instead of the distraction of out of pocket costs. Some of the benefits of the Critical Illness coverage include:


  • Once approved, benefit payments are sent directly to the enrollee
  • Coverage for such illnesses as cancer, heart attack, stroke, kidney failure and more
