Annual enrollment is your opportunity to learn about the 2022-2023 benefits offered by LATG, review your current coverages and choose the best options for you and your family. Please review our 2022-2023 Benefits Summary to learn more about our benefits offerings.
Open Enrollment begins today through July 18, 2022. Any changes you make will be effective August 1, 2022.
Please note, all employees MUST log into your Employee Navigator online portal to complete your enrollment.
Medical: With health care expenses continuing to rise nationally, we felt the impact of a significant increase in premiums from Humana. After reviewing and seeking other proposals, we were able to find a more reasonable option and thus will be making a move to United Healthcare Level Funded! United Healthcare’s network is as broad if not broader than Humana. All three options will be on a National Network platform. Please note there are some tradeoffs regarding benefits on the plans. United Healthcare Level Funded does not offer an “Ochsner Only Network” plan; therefore, we have replaced that plan with another alternative. If you elect the High Deductible Plan, please make sure to take advantage of a Health Savings Account (HSA). A Health Savings Account (HSA) allows you to set aside pre-tax dollars to help pay for qualified medical, dental and vision expenses. The funds can roll over from year to year, and you can take them with you when you change jobs or retire. An HSA is a separate account, independent of your medical plan. Think of it like a “healthcare IRA.” Depending on your circumstances, you may find that the High Deductible plan is a better option than the Copay Plans. LATG does not sponsor an HSA. Please reach out to your current banking institution, local credit union, or NFP (our benefits team), if you need help setting up an HSA.

Here are some nice features of the United Healthcare Level Funded plans:
Virtual Visits: A Virtual Visit through healthiestyou.com lets you see a doctor using the camera on your smartphone, tablet or computer. You can even get a prescription sent to your local pharmacy, all in 30 minutes or less. $0 per visit for members and dependents on the National Copay Plan. $45 per visit for members and dependents on the National High Deductible Plan.
Wellness: Last but certainly not least, you have access to an incredible wellness programs and resources included with the UnitedHealthcare medical.
· Motion – Walking program that gives you the opportunity to earn up to $1,095 per year!
· Rally – A fun, personalized digital experience designed to make getting healthy easier!
· Real Appeal – Free Digital Weight Loss Program that comes with a Success Kit, Transformation Coach, Online Group Classes, and more!
Voluntary Worksite Benefits: We will continue to offer Accident, Cancer, Critical Illness and Hospital Indemnity coverage through Guardian. These plans are designed to pay out cash benefits directly to you, and they also include wellness benefits which help you stay on top of your annual screenings. Please contact NFP benefits team if you need assistance in filing a claim.
Dental: We are happy to announce that we are moving to United Healthcare for our dental coverage! With this move, we were able to secure the same plan designs with cheaper premium.
Vision: We are moving to United Healthcare for our vision coverage as well.
SBC: I hereby acknowledge receipt of the Summary of Benefits and Coverage (SBC) found in Ease. I have read the SBC and am familiar with its contents. If I have any questions concerning the information, I will contact the carrier or my Human Resource Department to have my questions answered. I understand this SBC is not a contract, and the material represents guidelines subject to change.
Offer of Coverage: I certify that I have received enrollment and plan information for health care coverage that Louisiana Technology Group, Inc. offers, representing that it meets ACA requirements for affordable, minimum essential coverage, offering minimum value to me. I have been given the opportunity to enroll in or decline that coverage.
Pre-Tax Payroll Deductions: I authorize any payroll deductions required for the elections made in this online enrollment portal. I understand that these deductions will be made on a pre-tax basis unless I elect otherwise on this form. I understand that I cannot change my elections until the next open enrollment period, but I may change coverage for the dependents I am insuring or add new dependents if there is a "qualified" change in status. I have read the plan description and understand the post-tax deduction option.
