We want to make sure you’re getting the most out of our benefits—that’s why we’ve put together this Open Enrollment Guide. Open enrollment is a short period each year when you can make changes to your benefits. This guide will outline all the different benefits Frazee Construction offers, so you can identify which offerings are best for you and your family.
Annual enrollment is your opportunity to learn about the 2021 Health Care Program, review your current coverage and choose the best options for you and your family. Please review our 2021 Enrollment Guide and attend our upcoming education session to learn more about our 2021 benefits offerings
Enrollment forms are provided by the Human Resources department. If you are not electing coverage, you must submit a waiver form. These forms should be completed and returned to HR. Once you have made your elections, you will not be able to change those elections until the next open enrollment period unless you have a Qualifying Event (QE).
The benefits you choose during open enrollment will become effective from 12/01/2020 through 11/30/2021.
